Unfortunately, they're missing out on the full experience. I have a few friends who play cracked versions. In short, "Cracked Servers" are not safe. Without that authentification anyone is free to use any name they like regardless of it being used by someone else or not. Setting that setting to false basically disables the Authentication process that is done on the Mojang's servers for every user that connects to your server. Nobody else can use your name nor can you use a name that is already being used by another account. You login to the launcher with your Mojang account and since the game (Minecraft) is bound to your Mojang account it will autimatically detect it and sign you in with your Minecraft name. When online-mode is set to true, people can only use their minecraft name that they choosen when they purchased the game. While the "Premium Servers" have the setting online-mode set to true, the "Cracked Servers" have this option set to false. The only noticable difference is in the server.properties file. "Premium Server" as you would call it is actually no different to a "Cracked Server".
